ABDM Integration for
Modern Indian Clinics
The Ayushman Bharat Digital Mission is India's national digital health infrastructure. Over 90 crore citizens already have ABHA accounts. Relaya connects your clinic to this network with full FHIR R4 compliance, consent-managed record exchange, and automatic facility registration.
What is ABDM
Launched in September 2021 by the National Health Authority, the Ayushman Bharat Digital Mission creates a unified digital health ecosystem for India. It connects patients, providers, laboratories, pharmacies, and insurers through standardized protocols. Every citizen gets a unique ABHA (Ayushman Bharat Health Account) number that links their medical records across the entire healthcare system.

How Relaya connects you to ABDM
Six building blocks of the digital health ecosystem, fully integrated into your daily workflow. No separate portals, no manual data entry, no compliance headaches.
ABHA ID Verification
Patient shares their 14-digit ABHA number or scans their QR code at reception. Relaya verifies identity against the ABDM registry and auto-populates demographics. No form-filling required.
Health Record Exchange
Post-consultation, prescriptions, clinical notes, and diagnostic reports convert to FHIR R4 bundles and push to the patient health locker. Any provider with consent can access them.
Consent Management
ABDM consent artefacts are built into the workflow. Patient grants access, records flow. Patient revokes, connection closes. Full DPDP Act alignment with granular, time-bound consent.
Health Facility Registry
Relaya registers your clinic on the HFR with a unique facility ID. Your practice becomes discoverable on government health portals and the UHI network.
Unified Health Interface
UHI is the open discovery layer. Patients searching for nearby providers, booking appointments, or requesting teleconsultations can find your clinic through any UHI-enabled app.
FHIR R4 Compliance
All health data exchanged through Relaya conforms to the ABDM FHIR Implementation Guide v6.5. Structured resources for Patient, Encounter, MedicationRequest, DiagnosticReport, and DocumentReference.
Technical architecture
ABDM defines a layered architecture. Relaya implements each layer so your team never touches raw APIs or manages compliance certificates directly.
Identity Layer
ABHA number generation and verification via Aadhaar or mobile OTP. Demographic and biometric authentication. PHR (Personal Health Record) address linking.
Registry Layer
Health Facility Registry (HFR) for clinic registration. Healthcare Professionals Registry (HPR) for doctor verification. Both feed into UHI discovery.
Data Layer
HL7 FHIR R4 resources with ABDM profiles. ICD-10 coding for diagnoses. SNOMED CT for clinical terms. LOINC for laboratory observations. DICOM for imaging.
Consent Layer
Granular consent artefacts with purpose, time period, and data categories. Consent Manager integration for request-grant-revoke lifecycle. Full audit trail.
Exchange Layer
Health Information Exchange (HIE) for push and pull of records between providers. Encrypted data transfer with ABDM gateway mediation. Real-time status callbacks.
Discovery Layer
Unified Health Interface (UHI) for open network service discovery. Patients find providers, book appointments, and initiate teleconsultations through any participating app.
How it works with Relaya
Facility registration
Relaya submits your clinic details to the Health Facility Registry. You receive a verified HFR ID within one business day. Your practice appears on government health portals and UHI-enabled patient apps.
Doctor verification
Each practitioner at your clinic is verified through the Healthcare Professionals Registry using their registration council credentials. This links provider identity to every record they create.
Patient check-in
Patient presents their ABHA QR code or 14-digit number at reception. Relaya authenticates against the ABDM gateway, pulls verified demographics, and links the visit to their longitudinal health record.
Consultation and documentation
During the visit, your clinical notes, prescriptions, and orders are captured in the standard Relaya workflow. No additional steps. The system structures data into FHIR resources in the background.
Record push
After the consultation, FHIR-compliant health records push to the patient ABHA-linked locker. Prescriptions, diagnostic reports, and clinical summaries are all available for the patient or their next provider.
Cross-provider access
When the patient visits another provider, that provider can request your records through the ABDM consent flow. Patient approves, records transfer. Complete care continuity without phone calls or faxes.
Why this matters for your clinic
Government compliance
ABDM compliance is increasingly mandated for insurance reimbursement under PM-JAY, accreditation renewals, and government scheme participation. Being compliant now avoids disruption later.
New patient acquisition
Registration on the Health Facility Registry and UHI network makes your clinic discoverable to millions of patients searching for providers through ABDM-enabled apps. A free acquisition channel.
Zero duplicate paperwork
Patient demographics auto-populate from ABHA verification. Historical records pull from previous providers. No re-entering allergies, medications, or past diagnoses from scratch.
Care continuity
Your clinical records follow the patient across providers. Referral doctors see your notes. Specialists see your diagnostics. Better outcomes, fewer repeat tests, reduced liability.
Insurance integration
ABDM connects to NHCX (National Health Claims Exchange) for standardized insurance claim submission. Pre-authorization, claim adjudication, and settlement through digital rails.
Patient trust
Consent-managed data sharing gives patients control over their records. They choose who sees what, for how long. This transparency builds trust and strengthens the provider-patient relationship.
Standards and compliance
Relaya implements the full ABDM technical specification stack. Your clinic meets every interoperability requirement without managing certificates, API keys, or FHIR profiles directly.
