Each doctor clicks "Connect Google Calendar" in their Relaya profile. OAuth popup, authorize, done. 15 seconds per person.

Which calendars should block time? Just the primary? The personal one too? You pick. Personal events show as "busy" in Relaya without revealing details.

Forget about it
From this point on, it just works. Book in either place. Block time in either place. Cancel in either place. Everything stays in sync.
